Join the Association of Cancer Care Centers (ACCC) for this 2-part series designed to help oncology professionals understand and implement the Collaborative Care Model (CoCM), even in resource-constrained settings.
Session 1: Collaborative Care: A Solution for Providing Optimal Psychosocial Oncology Care
This training provides an introduction to the Collaborative Care Model (CoCM) and its role in enhancing comprehensive cancer care. Participants will explore strategic interventions and care coordination approaches that support successful implementation of collaborative care across a variety of cancer program settings. The training will also review the business case for CoCM, highlighting key considerations for demonstrating value, supporting sustainability, and integrating the model into cancer care delivery.
Session 2:
First Steps for Implementing Collaborative Care in Oncology
This training will guide participants through key considerations for planning and implementing a Collaborative Care Model (CoCM) within an oncology setting. Participants will learn how to define their program’s vision, scope, and target population; secure buy-in from oncology, social work, and organizational leadership; and establish essential roles, including care managers, psychiatric support, and program champions. The training will also explore the development of effective workflows, patient registries, and measurement-based care systems, as well as strategies for long-term sustainability, including billing and financial considerations.
